Enrolling in Treatment:

Complete the Enrollment Forms  

After the processing of your enrollment forms, you will be sent a link to the ITP Wellness Patient Portal.

First time clients: 1) Click the “Appointment Scheduling” tab below then click the “Patient Portal Access” tab. 2) Select provider Wesley Seabolt. 3) Select First Visit. 4) Select preferred date and time.  

Return clients: 1) Select provider Wesley Seabolt 2) Select Medication Return for a 20-minute appointment or Therapy Return for a 50-minute appointment. 3) Select the preferred date and time. 

Once appointment is set, you will receive a confirmation email (if not received, please check your spam folder). 

In order to participate in your treatment session just click the “Join Your Scheduled Appointment” tab on the home or patient portal pages.
